New Patient Registration
New Patients are Always Welcome
We are currently registering new patients. To register please come into the surgery and collect forms or register online.
To register online click the link below and download a registration form, fill it in and then email it back to us. To help provide us with information about you could you please also complete the New Patient Health Record Questionnaire.
If you wish you can pop into the surgery and the receptionist can provide you with a registration form to complete and hand in.
If you live in Troon, Loans, Dundonald, Monkton or Symington you may register with this practice.
We would ask that you provide some form of identification when you register ie passport, birth certificate, photographic driving license or national identity card.
Before registering with a new surgery it is helpful to ensure you have a few weeks worth of medication from your previous practice.
